How the Fortiva Model Works
Fortiva Wellness operates on a concierge membership model — one designed to make primary care feel the way it should: personal, unhurried, and built around you. Membership supports longer visits, direct access to your physician, specialist coordination, connection to community resources, and proactive care planning that keeps your health moving in the right direction.
Most major insurance plans, including Medicare, continue to apply for covered medical services — routine and telehealth visits, chronic care management, remote patient monitoring, annual wellness visits, laboratory work, imaging, preventive screenings, and other medically necessary care. These services are billed to your insurance as usual.
The membership fee is separate from insurance. It supports the time, availability, and physician accountability that traditional volume-based models simply don’t allow for — giving Fortiva the ability to keep patient volume intentionally small and focus on transparent, high-quality care delivered within all regulatory guidelines.
